Program Name: Forevify SMS Notifications

Program Description: By providing your mobile number through your senior living facility, you may receive SMS notifications from Forevify about your personalized song experiences, including invitations to contribute to community songs, notifications that your songs are ready to listen to, and replies from family members.

Message Frequency: Message frequency varies based on community activity and your participation. Most subscribers receive between 1 and 5 messages per month.

Message and Data Rates: Message and data rates may apply per your wireless carrier's plan.

Opt-Out Instructions: Reply STOP to any message to opt out of SMS at any time. STOP requests are processed immediately and permanently. Opting out does not affect your continued use of Forevify (still available via email) or any other service at your facility.
